Orthopedics

 

A medical discipline dealing with the diagnosis and treatment of musculoskeletal problems, i.e. diseases of bones, joints, muscles, ligaments and tendons. We treat congenital, developmental and acquired problems, e.g. due to trauma. We also provide services in the area of aftercare, i.e. physiotherapy.

TTA : a surgical procedure that has the best prognosis to date for treating anterior cruciate ligament damage. Our clinic was one of the first in the Czech Republic. Since 2006, we have operated on more than 550 knees using this method.

 

CWO : Surgical procedure to address anterior cruciate ligament damage. The indication for this procedure is where TTA is contraindicated.

Anterior cruciate ligament replacement: anterior cruciate ligament replacements in small breed dogs using various types of replacements. (Liga- Fiba, monofilament materials)

 

Elbow Dysplasia : Surgical procedures addressing unossified anconeal process, fragmented coronoid process, OCD of the elbow joint, Incongruity of the elbow joint

 

Ulnar osteotomy : Distal or oblique proximal osteotomy of the ulna for premature closure of the distal ulnar growth plate.

Corrective osteosynthesis: Correction of usually developmental defects: most often in the forearm and femur bones

 

OCD : Arthroscopic or arthrotomy treatment of osteochondrosis in the shoulder, elbow, knee, tarsal joints.

Arthroscopy is an examination and treatment method that allows the doctor to directly examine the joint and, using instruments, perform surgical procedures. The main benefit for the patient is minimal invasion, i.e. minimal damage from the surgical procedure. After an arthroscopic procedure, the patient usually leaves and uses the operated limb. The pain caused by the procedure is minimal. The operated joint heals faster with minimal postoperative scarring. As a result of minimal invasion, the risk of postoperative complications (e.g. infection) is also significantly lower.

 

Another important benefit is better visualization of the operated joint, i.e. more accurate diagnosis of joint disease. Better and more accurate diagnosis means a better prognosis for the patient.

 

During arthroscopy, 2-3 entrances, called ports, of 2-3 mm in size, are made into the joint. One entrance is always for optics, the second is for treatment, called the instrument port, and the third, smallest port, is used for flushing the joint. A needle with a diameter of approximately 1 mm is used for this port.

 

All large joints can be examined and treated. These are the shoulder, elbow and wrist joints on the thoracic limb and the hip, knee and ankle joints on the pelvic limb. However, this technique is commonly used for the shoulder, elbow and knee joints. It is rather exceptional for other joints.

 

Indications for arthroscopy:

 

Shoulder: OCD- Osteochondrosis, Instability, bicipital tendovaginitis, separated glenoid

Elbow: FCP- Fragmented coronal process, OCD

Knee: OCD, assistance in anterior cruciate ligament surgery.

TPO- triple pelvic osteotomy, DPO- double pelvic osteotomy

 

A unique surgical procedure that, if performed in a timely and correct manner, will allow a fundamental reversal in the development of hip dysplasia. It involves surgical reconstruction of one or both wings of the pelvis so that the hip joint is remodeled and excessive laxity in the joint does not lead to the development of significant osteoarthritis - Hip Dysplasia. This makes it possible to address even a very serious condition that would otherwise certainly lead to severe DKK.

 

We started performing the procedure in our office in 2003 and by 2015 we had performed more than one hundred and fifty of these operations with excellent results. The fact that TPO(DPO) permanently solves the condition of developing DKK has been truly confirmed.

 

However, it is important to know that this procedure cannot be performed on every patient. TPO(DPO) can be performed on dogs where the degree of development of secondary changes in the joint is only mild and therefore usually involves patients under one year of age. It is always necessary to correctly indicate the patient, eliminate the inappropriate and solve their condition in another way, e.g. with dartroplasty. In a correctly selected patient, the operated limb returns to function very quickly and usually the patient puts weight on the limb 3 days after the procedure. However, it is necessary to maintain strict rest for at least six weeks. After the pelvic bones have grown together, which is in two to three months, the hip joint and therefore the limb are fully functional and the patient can return to a completely normal life.

TTA

 

TTA-tuberositas tibiae advancement is a progressive method of treating anterior cruciate ligament (LCC rupture). We performed the first operation using this method at our workplace in 2006. We were one of the first two workplaces to use this technique. To date, we have operated on almost 600 patients.

 

The TTA surgical procedure was described by Slobodan Tepic and his colleagues in 2000. It uses the principle of changing the ratio of forces acting in the knee by shifting the tibial tuberosity forward. If this technique is performed correctly, the force damaging the anterior cruciate ligament is neutralized and it becomes unnecessary.

 

TTA is a very sophisticated and technically demanding solution for LCC rupture. It is therefore necessary to have appropriate technical equipment, but above all, the experience and professional quality of the surgical team. Experience and professional quality increase directly proportionally with the number of patients operated on in this way, as well as with systematic study. Therefore, MVDr. Zdeněk Ošmera, who performs TTA in our company, regularly attends educational events at KYONU in Zurich, the cradle of TTA, where it was created by Slobodan Tepic.

 

It should be emphasized that TTA is only one. In the last ten years, several other methods (TTA rapid, TTA2, Porous TTA, MMP) have been described, which are based on the described technique. None of the other techniques show better results than TTA alone. Therefore, there is no reason to use other techniques.

DARtroplastika

DARthroplasty

 

A unique surgical procedure for treating hip dysplasia. This technique involves reshaping the upper edge of the hip socket using bone grafts taken from the hip wing. The limb usually returns to normal function very quickly.

 

We started performing DARthroplasty at our workplace in 2005.
